IMPORTING RESTRICTIONS
COLLECT SPECIMEN SAMPLE
LABEL SPECIMENS PROPERLY
Each specimen should be clearly identified with the following:
- Animals Name
- Collection Date
- Specimen Type
BAG THE SPECIMENS
Place specimens in specimen bag ensuring the seal is tight. Only one animal’s samples may be submitted in each sample bag; although multiple sample bags may be sent to IDEXX in the same shipping box (as long as they fit). Print the requisition form sent by email. The requisition form may have multiple tests listed (if more than one was requested) but is animal specific. Fold the form in quarters as illustrated in figure 2. Place the form in the rear pocket of the specimen bag with the bar code facing out:
- Fold in half horizontally
- Fold in half again vertically
- Place form in specimen bag pocket
BOX THE SPECIMENS
Prepare the IDEXX-Direct shipping box as follows:
- Place the sealed specimen bag in an IDEXX-Direct shipping box (multiple specimen bags can be included in one box)
- Include a cold pack in a separate sealed bag in the box. Keep cold pack and specimen bags separate — do not insert cold pack into specimen bag; do not insert specimens into cold pack
- Do not include packing materials, i.e., no peanuts, newspaper, or bubble wrap
PACKAGE BOX AND SHIP SPECIMENS
Place the IDEXX-Direct shipping box into the FedEX® Large Clinical Pack (soft envelope with orange stripe) fold over and seal the package. Attach supplied FedEX® prepaid overnight shipping label to the package. Drop-off the package at a FedEX® Shipping location.
